- If your organization solicits donations from the public, Canada Revenue Agency registration business number is required
- Director Information, required for each director of the business
- Name, address, occupation including employer name and address
- Ownership Information, including full ownership structure with names and addresses
- Confirmation of business address, i.e., utility or property tax bill
- For each signing authority, one piece of original and valid government issued photo identification
- Partnership Agreement
- Certificate of Business Registration (if applicable)
- Trade Name Registration (if applicable)
- Partnership/ownership information, including full ownership structure with names, addresses, occupations including employer names and addresses
- Confirmation of business address, i.e., utility or property tax bill
